Winter Sports Information
Winter Sport Physical Packets and Recertification Forms. In short, if your athlete for the winter has not played a fall sport they need the full packet completed; if they have, they only need the recertification form. They may take forms to their personal doctors on their own or attend our one-day physical opportunity on Monday, November 7th from 9-11:30am - each physical is $15.00.
